Master of Science in
Information Systems
Administrative forms are available for
students to download, print, and complete. Refer
to the accompanying policies and procedures for the use and
completion of them. Upon completion, submit the signed
form and appropriate materials to the Graduate Programs
Office in 1360 Gerdin Business Building.
Application for Graduate Assistantship
On occasion, assistantships become available during the
academic year. To be considered for a position, submit
the application to the Graduate Programs Office in 1360
Gerdin Business Building.
Application for Graduation - This form
must be submitted by the end of the first week of the semester
(fall or spring) in which the student expects to receive the
degree or by the last day of the spring semester when wishing to
graduate during summer session.
Program of Study Form- This form should be completed
as soon after the committee is formed as possible.
Committee Appointment Form - It is
recommended that the committee be formed as early as the second
semester of graduate study. No later than the term before
the final oral exam or preliminary oral exam.
Request for Final Oral Exam - This form can
be picked up at the Graduate College or 1360 Gerdin Business
Building. The form must be submitted at least 3 weeks before
your scheduled final oral exam. Last day for your final oral
exam is approximately 3 weeks before graduation.
