Forms used by full-time MBA students

MBA Forms

Administrative forms are available for students to download, print, and complete. Refer to the accompanying policies and procedures for the appropriate use and completion of each form. Upon completion, submit the signed form and appropriate materials to the Graduate Programs Office in 1360 Gerdin Business Building.

Petition to Enroll for Independent Study
To request enrollment in an Independent Study within the College of Business, complete this form. A maximum of three credits of Independent Study, 590, can be applied toward an MBA.

Petition for Variance of Established Requirements
This form is used to request a variance from an established set of guidelines for a particular program. Such requests may include: 

  • Request to approve elective credit from outside of the College of Business

  • Request for substitution of a required course

  • Request for waiver of a required course

Application for Graduate Assistantship
On occasion, assistantships become available during the academic year. To be considered for a position, submit the application to the Graduate Programs Office in 1360 Gerdin Business Building.

Internship/Co-op Learning Contract

Graduation Forms
The following  forms must be completed by the mid-point of the semester prior to your anticipated graduation. Several of the forms have been completed with basic information for the MBA degree. Print, complete, and submit these forms to the Graduate Programs Office in 1360 Gerdin Business Building by the eighth week of the semester prior to your anticipated graduation.
